Project Details An innovative scheme designed to improve efficiency in healthcare delivery and release hospital capacity for patients with more complex and urgent needs. The scheme introduces extended primary care services in the specialities which most require additional capacity to achieve the 18 week waiting times target (from referral to treatment). Job description This is a new post created for the Greater Manchester NHS which will commence in early 2009. The aim of the scheme is to treat NHS patients who would traditionally have been in secondary care but will now been seen closer to home in a community setting within a four week timeline. There are three exclusion criteria , patients under the age of 18 years, clinical emergencies and patients with high suspicions of cancer at the time of referral. Patients will be referred to the scheme by their GP through patient choice and the NHS Choose and Book system. The referral process will incorporate default pathways via Map of Medicine. The working culture is one of a professional, multidisciplinary team work an emphasis on high quality of service, with particular reference to a safe, efficient, evidence based service; structured around the patient. We view an excellent patient experience as a key determinant in designing the service. The appointee will have a proven track record in delivering high quality care and be able to demonstrate how this impacts upon a positive experience for all patients and staff The scope of work will include paper clinical triage of referrals from General Practitioners, face to face clinical assessment of patients, treatment planning and onwards referral to secondary care if indicated. Minor procedures suitable for an out patient clean room environment as per HCC guidance will be performed. This may include for example: EXAMPLES Podiatry procedures including removal of toenails Injection of joints Aspiration of knee/joints THE MOBILE CLINICS The scheme will consist of two mobile clinics covering a total of seven locations in the Greater Manchester area – Bolton, Rochdale, Denton Oldham (North Manchester) and Salford, Stretford and Longsight (South Manchester). In addition to this there will be one Endoscopy Unit and a Patient Management Centre. In addition to assessments the clinics will be supported by the full spectrum of diagnostics inclusive of MRI, CT, X-ray and Ultrasound. Supportive therapy that will be available includes Physiotherapy, Occupational Therapy, Audiology, Podiatry and Speech and Language Therapy. TRAINING OF NHS MEDICAL, NURSING AND ALLIED HEALTH PROFESSIONALS The service is a designated training facility for NHS Medical, Nursing and Allied Health Professional staff. Consultants will be accredited as trainers for junior medical staff as agreed with the deanery and training will be provided on a placement basis. Appropriate training opportunities for Junior Doctors will be decided by the postgraduate deans on behalf of the PMETB and is not envisaged that this training will commence until late 2009. UNEXPECTED EMERGENCIES Appointees must be able to demonstrate their skills in managing unexpected clinical emergencies that may arise during the course of their shift. CASE CONFERENCE The appointee maybe requested to participate in case conferences and discussion for patients with urgent needs for medical attention. MANAGEMENT STRUCTURE FOR THIS POST This service will have a management team of Clinical Lead and Unit Manager. There will also be a support team of Specialist Nurses, General Nurses and Health Care Assistants. KEY WORKING RELATIONSHIPS Internally: The Lead Nurse, Clinical Lead and Manager for the Centre Consultant colleagues in own and other specialities Members of the nursing and allied health professional team Other members of the Patient Management Centre support team Other Clinicians within Care UK primary and secondary care divisions Care UK Medical Director Externally: Secondary care colleagues in own and other specialities Professional bodies Referring General Practitioners Community Health Structures Deanery HOURS OF WORK A full time post will be 40 hours per week (10 sessions of four hours). The appointee will be expected to work a shift system Monday – Sunday and travel to the unit at the set locations. Of the 4 hours; 3,5 hours will be allocated to direct patient care and 0.5 hours for supporting administrative activities. These roles can be either part of full time, from a minimum of 2 sessions per week, up to a maximum of 12 sessions. Final job plans will be discussed and agreed with the successful applicant and will include a combination of social and unsociable clinical hours. Unsociable hours may include evenings and weekends as it is envisaged that the service will be seven days a week starting at 07.45 and finishing at 20.30. WORKLOAD FIGURES It is envisaged that the service will conduct 8-10 new patient consultations per session. ADMINISTRATIVE SUPPORT The appointee will undertake administrative duties associated with the running of his/her clinical work. He/she will have administrative support from the Patient Management Centre and access to a PC and the Internet for work purposes. All clinical records will be managed through a paperless clinical software solution. As the handover at interface between services is a key risk for patients, it is the responsibility of the consultant to ensure all records are accurately maintained on the clinical software system whilst adhering to the timelines of onward referrals (2d). In the event of a clinical emergency; it is expected the postholder will lead on the safe clinical transfer of the patient. HEALTH CLEARANCE Any offer of employment will be conditional on satisfactory health clearance by Occupational Health. This is usually by questionnaire but may involve a medical examination. Principal Duties and Responsibilities • Take ongoing responsibility for patients under his/her care. • Participate in the development of the Orthopaedic surgery service, treatment protocols and guidelines and the greater knowledge in the management of conditions. • Take a lead in the management of patients. • Develop and maintain collaborative relationships with medical colleagues in other specialities and participate in regular clinical meetings. • Take responsibility for the professional supervision and development of trainee doctors within the speciality. • Develop and maintain good communications with General Practitioners and colleagues from acute hospitals. • Demonstrate firm involvement in clinical governance, risk management and clinical audit. This will include the development and maintenance of appropriate systems and practices to ensure continuous safe clinical practice. • Share responsibility for data protection arising out of the use of computers. • Comply with all Policies and Procedures. • You will be expected to participate in all aspects of clinical governance. The appointee is required to participate actively. • The post holder will ensure that all services are provided in an effective and efficient manner and that the principles of risk management and clinical governance are maintained at all times. Essential Experience • Full current GMC registration on appropriate specialist Register. • Evidence of experience and expertise in relevant clinical speciality. • Ability to communicate effectively with clinical colleagues, other specialties and support staff • Fluent in spoken and written English |
