This is a new post created for the Greater Manchester NHS which will commence in June 2009. The scope of work will include paper clinical triage of referrals from General Practitioners, face to face clinical assessment of patients, treatment planning and onwards referral to secondary care if indicated. Procedures suitable for a clean room environment as per HCC guidance will be performed. The role will consider: Pre-operative assessment for surgery as per NICE guidance Conscious sedation for endoscopy patients No general or regional anaesthesia will be administered within this service The Mobile Clinics The scheme will consist of two mobile clinics covering a total of seven locations in the Greater Manchester area – Bolton, Rochdale, Denton, Oldham, Salford, Stretford and Longsight. In addition to this there will be one Endoscopy Unit and a Patient Management Centre. In addition to assessments the clinics will be supported by the full spectrum of diagnostics inclusive of MRI, CT, X-ray and Ultrasound. Supportive therapy that will be available includes Physiotherapy, Occupational Therapy, Audiology, Podiatry and Speech and Language Therapy. The mobile units is supported through a central Patient Administration Centre based at Piccadilly, Central Manchester Management Structure for this post The service will have a management team of Clinical Lead and Unit Manager. There will also be a support team of Specialist Nurses, General Nurses and Health Care Assistants. The appointee will be managerially accountable to the Unit Manager and professionally accountable to the Medical Director for Care UK. Key Working Relationships Internally: ■ The Lead Nurse, Clinical Lead and Manager for the Centre ■ Consultant colleagues in own and other specialities ■ Members of the Nursing and Allied Health Professional team ■ Other members of the Patient Management Centre support team Externally: Secondary care colleagues in own and other specialities Professional bodies Referring General Practitioners Community Health Structures Deanery Hours of Work A full time post will be 40 hours per week (10 clinical sessions of four hours). The appointee will be Expected to work a shift system Monday – Sunday. Of the 4 hours; 3,5 hours will be allocated to direct patient care and 0.5 hours for supporting administrative activities. Workload Figures It is envisaged that the service will conduct 8-10 new patient consultations per session. Over and above this; it is expected that patients previously attended to will be followed up as clinical appropriate; either by face-to-face or telephonic appointment. There is no on-call requirement. It is however expected that the consultant will be primarily responsible for the integrated and ongoing care of the patient until discharge. Administrative Support The appointee will undertake administrative duties associated with the running of his/her clinical work. He/she will have administrative support from the Patient Management Centre and access to a PC and the Internet for work purposes. All clinical records will be managed through a paperless clinical software solution. It is the responsibility of the consultant to ensure all records are accurately maintained Health Clearance Any offer of employment will be conditional on satisfactory health clearance by Occupational Health. This is usually by questionnaire but may involve a medical examination. Principal Duties and Responsibilities • Take ongoing responsibility for patients under his/her care. • Participate in the development of the Anaesthesia service, treatment protocols and guidelines and the greater knowledge in the management of conditions. • Take a lead in the management of patients. • Develop and maintain collaborative relationships with medical colleagues in other specialities and participate in regular clinical meetings. • Take responsibility for the professional supervision and development of trainee doctors within the speciality. • Develop and maintain good communications with General Practitioners and colleagues from acute hospitals. • Demonstrate a firm involvement in clinical governance, risk management and clinical audit. This will include the development and maintenance of appropriate systems and practices to ensure continuous safe clinical practice. • Share responsibility for data protection arising out of the use of computers. • Comply with all relevant Policies and Procedures. • Participate in all aspects of clinical governance. The appointee is required to participate actively. • The post holder will ensure that all services are provided in an effective and efficient manner and that the principles of risk management and clinical governance are maintained at all times Desirable Qualifications • Experience of teach junior medical staff and students. • Ability to teach clinical operative and practical procedures. • Appropriate accreditation with relevant Deanery as a trainer. Essential Experience • Full current GMC registration on appropriate specialist Register. • Evidence of experience and expertise in relevant clinical speciality. |
