You will be working with one of our clients teams based in hospital trusts. The post holder treats a specialist caseload of NHS patients who remain under the care of their consultant but who return home to complete their treatment. Job Summary: Undertake patient assessments on hospital sites and provide acute nursing care to patients in their own homes The role includes some evening and weekend work, including participation in an on-call rota This role is ideal for a forwardthinking and enthusiastic nurse who is happy travelling to patients' homes and relishes the opportunity of working autonomously. Key responsibilities: Deliver at all times high quality nursing care by assessment, implementation and evaluation of patients’ needs. Maintain and support harmonious working relationships within the organisation. Act in such a manner as to satisfy public trust and confidence, to uphold and enhance the good standing and reputation of the nursing profession. Maintain accurate documentation at all times adhering to NMC standards and company protocols Establish and promote sound lines of communication to all staff, patients and hospitals. Supervise the work of junior colleagues. Work with the medical staff in order to ensure positive outcomes for each patient episode. Formulate, in conjunction with senior staff, procedures and standards, and to participate in clinical and organisational audit. Ensure adequate equipment/supplies to undertake patient visits. Participate in staff appraisal and the identification of professional needs. Set and evaluate personal objectives. Participate through mentorship in the orientation and development of staff. Participate in “in-house” training sessions, and attend meetings and discussions when required. Utilise all resources effectively in order to deliver quality care at all times. Be actively involved in nursing research projects in support of clinical practice. General: Be personally accountable for own actions and omissions in line with the NMC Code of Professional Conduct and to take every opportunity to maintain and improve knowledge and professional competence. Ensure compliance with all corporate policies, local policies and legal requirements relating to clinical care, Health and Safety and the storage of drugs. Ensure that equipment is handled correctly and maintained in safe working order. Maintain and develop a harmonious working relationship within MediHome and with other external agencies. Ensure that confidential information is not disclosed, including information about staff and patients, to any unauthorised member of staff, or to anyone outside the employment of the organisation who is not authorised to such information. This job description is intended to reflect, in outline, the envisaged responsibilities of the postholder, which may change over time with the corporate and local business needs of the company. It will be subject to periodic review, as a minimum annually. R.G.N. 1st Level Evidence of continuing professional development At least 3 years post registration/adaptation experience Current experience with venepuncture Experience of working on own Ability to communicate well, verbally and in writing Ability to work well as part of the team and be flexible Desirable: ENB 998/ C&G 730 or equivalent. ENB community course and/ or equivalent experience. Previous community care experience and experience in an acute care environment. (ITU, A&E, etc.) Current experience with cannulation Wound care and associated products experience. Current experience with IV treatment/therapy Ability to demonstrate own initiative and thinking Flexibility Able to cope with ambiguity Excellent communication/interpersonal skills Time management Skills Able to work independently and carry out assigned tasks in a changing demanding environment Previous experience at managing own case load independently Reliable work record Time keeping record Computer Literate Nurses - £26,000 - £31,000 p.a.(depending on location) plus bonus, healthcare benefits and childcare benefits (Unsocial hours supplements apply 20.00 – 23.00 and to weekend working) |
