Nurse Practitioner - Out of Hours

This organisation is a leading independent provider of health and social care service solutions. Services include residential care, homecare, acute care, primary care and others. You will work as part of a team of Nurse Practitioners within the Medical Centre. Establish inter-agency working where necesssary, to ensure good communication increases quality of care for patients. Ensure that you own clincial practice is consistent with autonomous practice within the role. Job Details To be able to work autonomously and part of the team Be able to assess, diagnose and provide health advice Suture and plaster, deal with minor illness and injury care Medicine management/advice and prescribing Referral to other health professional or service as required Set standards for care by implementing protocols etc Develop new and innovative tools to assist patients and those supporting their care Maintain accurate documentation of assessment, planning and treatment of patients Qualifications Recognised NP qualification to degree level or equivalent Teaching/mentoring experience and/or qualification is desirable 5 years post registration experience Evidence of CPD Skills/Experience Amongst othere you will have the following: Experience in management of long term conditions eg asthma, COPD, diabetes Proven ability to evaluate the safety and effectiveness of your own clinical practice Previous experience in managing and developing a nursing team. Knowledge of NICE guidelines, National Service Frameworks etc Be a reflective practitioner Excellent computer skills Able to maintain effective communication pathways Good time management and ability to prioritise workload Able to think strategically Benefits Competitive Hours of work: ND, weekends and Bank Holidays
