DAY SURGERY CENTRE Patient and Clinical Area Description Our client's Surgery Centre is one of the largest, purpose built, stand-alone units in the UK. Their unit is at the leading edge in developing and expanding the traditional boundaries of day surgery practice. They have a diverse and multicultural client base and as a team are dedicated to providing the highest quality of care for their patients. They have close working links with the British Association of Day Surgery [BADS] and have been involved in piloting new surgical and technological innovations. The current Day Surgery Centre opened in December 1991 and in 2005 they completed an extensive £4.6 million expansion and refurbishment programme, which has increased their capacity to 7 theatres (two of which have integrated STORZ OR1 theatre systems), with upgraded post-operative facilities and a dedicated pre-assessment suite, ophthalmology unit and paediatric self contained area. The unit is open from 07:15 till 20:00 in the evening, Monday to Friday. They also open on selected Saturdays for NHS and private patient initiatives. Day Surgery at this hospital is equipped to cater for all ages from children (separate facilities) through to adults (no upper age limit) and for patients with learning disabilities. They undertake rountinely the following surgical specialities: • General and Colorectal surgery • Laparascopic procedures such as Cholecystectomy, hernia repair • Trauma & Orthopaedics, including ACL reconstruction, complex shoulder surgery • Vascular surgery • Maxilla-facial surgery • Gynecology, including laparoscopic & hysteroscopic resections • Ophthalmology, including day case vitrectomy and corneal surgery • Urology, including day case PVP laser prostatic surgery and laparoscopic • Paediatric General Surgery, Endoscopy and Bronchoscopy • Neurosurgery, including VNS surgery for epilepsy & in Cardiac, Cardioversions • Renal, AV fistula formation procedures “Day Surgery” care begins before the patient comes into hospital and finishes when the patient no longer requires support. Their patients stay from a few hours up to eight hours depending on the complexity of the surgery performed. They provide a nurse-led pre-assessment clinic where patients receive written and verbal information, are able to discuss their concerns and are assessed as to their physical and social suitability for day surgery. This service is offered both face to face and by telephone. Our cleint operates the booked admissions programme and aim that 100% of their patients are able to mutually agree their pre-assessment and operation dates on the day of decision to treat. Unit Philosophy In the Day Surgery Centre they believe that their aim is to deliver safe, effective and efficient nursing and medical care by staff trained in the principles of day surgery. We will maintain a clean, comfortable and welcoming environment within the centre, and their staff will be friendly, approachable and professional. The patient will always be treated as an individual. They will respect their cultural and religious beliefs, and make every effect to provide support for anyone having difficulties with the language. Each patient will be the concern of an individual nurse from admission to discharge. They believe in a team approach, and recognise that each team member, whatever their role, can contribute to the development of patient care. Communication is recognised as important within the team, and involves both hospital professionals and our community partners in the primary sector. The staff will ensure that patients, and where appropriate relatives and friends, receive the information and the physical and emotional support needed to cope with the effects of any procedure. Their goal is to enable all our patients to return to an independent life at home safely and as quickly as possible. Staffing There are a total of 70 Clinical practitioners posts [nursing and ODA / ODP qualified as well as trained theatre HCAs and support workers]. The team is lead by a Senior Nurse who is supported by a team of Clinical Co-ordinators [Band 7] and Team Leaders [Band 6]. Each of our senior team are encouraged to develop an individual ‘portfolio’ of practice. This includes undertaking daily responsibility for a specific clinical area and co-ordinating staff, resources and practices within their sphere. The Day Surgery senior grades operate an on-call pager advisory service 24/7. The service is available to patients and carers overnight and at weekends to deal with patients’ problems and queries. Within the main team they encourage all of their practitioners to become multi-skilled and to work throughout the unit in some or all of the following day surgery specialities: Theatre, Anaesthetics, 1st stage recovery, 2nd stage recovery [ward] and pre-assessment. All Staff are actively involved in examining existing practices, facilities and patient information literature and helping to redesign the ways we work. To facilitate this they have regular team meetings and structured training days throughout the year. They believe the key to their success and the high standards of care they deliver are communication and teamwork and they always welcome applications from enthusiastic and caring individuals, for full or part-time hours whom would enjoy working in this challenging and fast moving environment. As a department they operate flexible working initiative, so you can balance work with personal commitments. Central to supporting the clinical staff, Day Surgery also comprises of a team of 12 admission staff and their role is vital to the successful operation of the department. They are based within the centre and are integral to the pre-assessment service. Education and Support Within Day Surgery their training and development vision is to enable the development of a competent, knowledgeable and flexible team. Their Practice Development Nurse is available to support all new and existing staff and we also operate an appraisal system to facilitate all staff to develop, have an individual professional and personally plan, and ultimately to progress within the unit. To facilitate staff to develop the clinical skills and knowledge in relation to day surgery they have a comprehensive series of competency documents that help underpin the learner’s progress. This runs in conjunction with their internal rotational training programme that gives the individual innumerable opportunities to gain experience in all areas of clinical practice. In addition as a large teaching hospital they can also offer you excellent educational opportunities – from Trust run in-house study courses to postgraduate education through the Nightingale Institute. These opportunities span the needs of the newly qualified [mentorship and teaching & assessing courses], clinical skill courses such as the anaesthetic or theatre course, right through to leadership courses. On going developments Within Day Surgery thye are in an exciting period of change in which the traditional views on inpatient surgical practice are being challenged. they have already at this hospital been able to offer patients who previously would have had to stay 2-5 days as an in-patient, a day case option in the areas of gall bladder surgery, ACL reconstruction, shoulder surgery and prostatic surgery. Our client Hospital has a strategic vision to reconfigure how as a Trust it will deliver surgical services in the 21st Century. They are striving to achieve the default position for all patients who are planned to have one of the audit commissions’ baskets of procedures [minor/intermediate surgery] to be a day case automatically and only admitted as an in-patient where day surgery is contra-indicated. This is now recognised nationally in the Department of Health’s vision of treating day surgery as the norm [rather than as in-patient] for elective surgery [10 High Impact Changes, 2004]. They are currently implementing a rapid access surgical service in conjunction with the First choice programme at this Trust. This means that patients presenting in A&E requiring minor surgical procedures can access the unit for day surgery and in the interim go safely home rather than having to be admitted. Our initial findings indicate high patient satisfaction and this focus on the patient’s experience underpins all of our developments. Sounds interesting? Would you like to be part of the team?
